The Middletown Select Soccer Club is happy to invite you to come and spend the weekend of Sept 23-24, 2017 at the MIDFEST SOCCER CLASSIC TOURNAMENT. The tournament will be held at Smith Park fields in Middletown. Smith park has been home to many tournaments: including the Middletown Spring Blast, SAY Franchise, State SAY and High School Tournaments.
The tournament will offer competition for boys and girls in the 9U through 15U brackets. All teams must be in good standing with the United States Soccer Association and only players with valid player passes will be eligible to play. 8U will be accepted in the 9U division. A maximum of 14 players per roster of 9U, 10U 7v7 division and a maximum of 16 players, 11U-12U 9v9 division. A maximum of 18 players per roster will be allowed for 13U-15U 11v11.
Divisions will be made as long as there are 4 teams to be placed in the schedule
Four (4) guest players will be allowed. No player may play on nor be registered on more than one participating team in the tournament. Tournament Committee reserves the right of acceptance, denial or placement of any team.
The entry fee of $475.00 for 8U and 10U and $525.00 for 11U-15U per team should be mailed along with the enclosed application form to the address listed below by August 20, 2017. Teams not accepted will have their application fee returned in their notification letter. No entry fee will be returned to those teams accepted into the tournament. Notifications will be emailed and posted on the web site by September 1, 2017
IMPORTANT ISSUE: CONCUSSION LAW
In order to protect young athletes, the State of Ohio has passed a concussion law, commonly called the “Return to Play” law. Therefore, all Ohio Tournaments are responsible to monitor and track compliance with the concussion training requirements of the Return to Play law. Each coach (head coach and assistants) must receive approved concussion training prior to April 26, 2013. See O.R.C. 3707.511(C)(1). Tournaments are required to collect and maintain certificates of compliance from the coaches.
1. All coaches MUST complete the Concussion Training Course from either NFHS (preferred) or CDC and submit a COPY of the completed certificate to the tournament. Both online courses are FREE of charge! See the links for the Concussion Form after you CONFIRM acceptance (using your midfest #) on the forms list.
2. Coaches must also carry their certificate COPY of compliance with them during the event.
3. Coaches are also required per Ohio law, to have distributed a COPY of the Parent Concussion Information Sheet to the parents of their team players.
If these 3 items are not satisfied, those coaches will NOT be able to coach at this event.
If a player is believed, by either a coach, referee or tournament official, to exhibit the signs, symptoms or behaviors of concussion during the tournament, that player shall be prohibited from continuing to play that same calendar day and may not play until they have received clearance to “return to play” by a physician or other authorized medical personnel.
We are very proud of the Soccer facilities at Smith park in Middletown. Smith offer playgrounds, walking trails around the park, refreshment areas and with a total 28 fields for your soccer enjoyment. There are plenty of restaurants and shopping close park. Pets are allowed at Smith park but must be kept on leash and picked up after. The tournament is set up on a three game format. Individual and team trophies will be awarded for first and second place teams in each division. Trophy's will be awarded after final game at the stage near the tournament headquarters. brackets are listed as Red(Upper), White(Middle), Blue(Lower), Yellow(lower). We do the best we can to bracket teams and try to bracket teams as the appear in league divisions. Some teams may go up or down a division at the discretion of the scheduling committee.
Parking will be limited in some areas of the park, so please carpool as much as possible.
Team Registration will be held on Saturday morning before your first game at Smith Park at Registration Tent
1 coach or Team admin is required to report to the Registration Tent 45 minutes before your first game to register your team
and pick up team packet
ALL GUEST PLAYER CARDS MUST BE STAMPED AND PE-APPROVED BEFORE SATURDAY MORNING,
GUEST PLAYER CARDS WILL NOT BE APPROVED AT TOURNMAMENT HEADQUARTERS.

2 copies of roster (we will keep 1 copy you will keep 1)
2 copies Change or add rosters(if applicable) (we will keep 1 copy)
1 copy of Middletown Player Release of Liability form. (we will keep ) needs all parent signatures
1 copy of Team contact form. (we will keep)
Medical release forms will be required for all players (you will need to show these and keep with you)
Permission to travel(If applicable)
Copy of coaches concussion certificate (you will keep these and show them)
